VA GERD Evidence Guide: 2024 Revised Criteria, Endoscopy, and Stricture

In May 2024, the VA modernized the digestive rating schedule under 38 CFR 4.114. For decades, GERD was rated by analogy to hiatal hernia (DC 7346) based on vague terms like 'pain, vomiting, and hematemesis.' The updated schedule establishes dedicated Diagnostic Code 7206 for GERD and esophagitis, tying disability percentages directly to objective structural findings and clinical interventions.

Under the new DC 7206 criteria, compensable ratings above 10% depend on documented esophageal stricture. A 10% rating is assigned for symptoms requiring daily medical management with acid suppression therapy. A 30% rating requires documented stricture requiring dilation at least once a year. A 50% rating requires recurrent strictures requiring dilation at least twice a year.

The highest scheduled evaluation under DC 7206 is 80%. This tier is reserved for severe, refractory esophageal disease causing near-total mechanical obstruction, profound dysphagia, and substantial weight loss requiring feeding tubes or intensive parenteral nutritional support. Complete medical documentation of nutritional metrics and specialist consultations is essential.

Subjective complaints of heartburn and acid reflux are no longer sufficient to secure higher ratings under DC 7206. You must submit objective imaging and endoscopic proof: esophagogastroduodenoscopy (EGD) reports showing mucosal erosion, Barrett's esophagus, or esophageal ring formation, along with barium esophagrams demonstrating stricture caliber.

Veterans who received a service-connected GERD rating prior to May 19, 2024 under legacy DC 7346 are protected. Under 38 CFR 3.951, the VA cannot reduce an existing rating simply because the diagnostic criteria changed, provided the veteran's condition has not materially improved. However, claims for an increased rating will be adjudicated under the new DC 7206 criteria.
