How VA Rates Chronic Sinusitis: DC 6510-6514 Episode Frequency

Sinusitis is evaluated under DC 6510 through DC 6514 (ethmoid, frontal, maxillary, sphenoid, or pansinusitis) using a shared rating scale: 0% for sinusitis detected by X-ray only; 10% for one or two episodes per year requiring prolonged (4 to 6 weeks) antibiotic therapy, or 3 to 6 episodes requiring 1 to 2 weeks of antibiotics; 30% for three or more prolonged antibiotic episodes per year, or more than six episodes requiring 1 to 2 weeks of antibiotics; and 50% for radical surgery with chronic osteomyelitis or near-continuous episodes requiring continuous antibiotic therapy.

An incapacitating episode is defined as an acute flare-up that requires bed rest and treatment by a healthcare provider. Non-incapacitating episodes involve acute symptoms such as headaches, facial pain, and purulent nasal discharge that are treated with shorter courses of antibiotics without required bed rest.

Chronic sinusitis is an established presumptive condition under the PACT Act for veterans who served in covered Persian Gulf, Iraq, Afghanistan, and associated airspace theaters. A formal diagnosis of chronic sinusitis with symptoms lasting longer than 12 weeks qualifies for presumptive service connection.

Medical records must document the exact frequency of episodes and the duration of each antibiotic prescription. A CT scan of the paranasal sinuses showing mucosal thickening or sinus opacification, surgical operative reports, and treatment notes from an otolaryngologist (ENT) provide the strongest evidence.

Under 38 CFR 4.14, a veteran diagnosed with both chronic sinusitis and allergic rhinitis can be awarded separate disability ratings for each condition. Because sinusitis affects the paranasal sinuses and rhinitis involves the nasal passages and turbinates, the two diagnoses represent distinct anatomical pathologies. The VA combines the two individual evaluations using standard VA combined rating math.
