VA Hypertension Evidence & C&P Exam Guide: Blood Pressure Readings and DC 7101

Under 38 CFR 4.104, Note 1 explicitly dictates the diagnostic requirements for hypertension claims. A diagnosis of hypertension requires blood pressure readings taken on two or more different days, or twice daily across three consecutive days. Single isolated readings taken during an emergency room visit or acute illness cannot legally establish a valid diagnosis for VA compensation purposes.

Hypertension is evaluated under DC 7101 based primarily on diastolic blood pressure: a 10% rating is awarded for diastolic pressure measuring 100 to 109 mm Hg, or systolic pressure of 160 to 199 mm Hg; a 20% rating is granted for diastolic pressure of 110 to 119 mm Hg, or systolic of 200 mm Hg or higher; a 40% rating requires diastolic pressure of 120 to 129 mm Hg; and a 60% rating requires diastolic pressure of 130 mm Hg or higher.

Veterans whose blood pressure is medically controlled within normal ranges often fear their rating will be reduced to 0%. Under DC 7101, a 10% rating is explicitly authorized if there is a documented medical history of diastolic pressure predominantly measuring 100 or more that currently requires continuous daily antihypertensive medication for control.

The PACT Act added essential hypertension to the list of statutory presumptive conditions for veterans exposed to Agent Orange and tactical herbicides in Vietnam, Thailand, Guam, American Samoa, and Johnston Island. Under 38 CFR 3.309(e), eligible veterans do not need to prove an in-service injury or event; showing qualifying service and a current clinical diagnosis satisfies service connection.

During a C&P exam, the examiner takes three separate seated blood pressure readings separated by several minutes. If those readings do not satisfy diagnostic criteria, the examiner must order outpatient blood pressure monitoring on consecutive days. Ensure your claims file contains pharmacy printouts demonstrating continuous prescription antihypertensive therapy.
